About the role:

An exciting opportunity to be involved in service delivery development, we are looking for the right individual to lead a vital new pilot project based in a frontline healthcare setting.

The Social Prescribing Link Worker Service takes a holistic approach to an individual’s health and wellbeing, connecting people to community groups and statutory services for practical and emotional support. In this position, you will be based in the Emergency Department (ED), supporting people to access appropriate care pathways, improving their health outcomes, and reducing the burden on the emergency care department.

You will work closely with health professionals within the NUH Emergency Department to take referrals and identify people who bypass primary care and use ED for non-urgent care needs. The role will involve promoting the benefits of social prescribing using preliminary holistic conversations, signposting to appropriate organisations and connecting with SPLWs based in the community, with the aim of reducing the persons likelihood to fall between the gaps of support services and/or continuing to use the emergency department inappropriately.

The successful candidate will possess excellent organisational, interpersonal and IT skills, and be able to work independently, as well as part of a team, working closely and collaboratively with SPLW colleagues in the community and other health professionals. With an awareness of the relevant issues, you will also be empathetic, sensitive and able to motivate and empower others. Experience of providing Social Prescribing support would be advantageous, but is not essential.

Hours of work:

15 Hours Per Week. Fixed term until approximately 28 October 2022.

Based: Emergency Department in Queens Medical Centre
