Hours: 37 hours per week (job share considered)
Salary: £22,972.10 per annum (pro rata)
Based: Response Centre – Ashfield, Mid Notts
Tenure: Fixed term for 12 month

About the role
The “Community Response Service” is a new and innovative collaboration between Nottingham & Nottinghamshire Integrated Care System and Age UK Notts to improve the quality of care of older people in the community, keeping people safe and well at home across Bassetlaw, Nottingham City, and Nottinghamshire County.

We have an exciting opportunity for a passionate and resilient individual to deliver a high-quality service. Co-located with the “Urgent Community Response” team (Mid Notts or City) you will help identify and triage referrals for our Age UK Notts community response support workers and volunteers to provide home response, safe and well checks and community signposting to support people to remain at home.
