We are very pleased to announce we are looking for an experienced Domiciliary Care Co-Ordinator to join our team.

We are looking for a Care Co-Ordinator to work alongside our existing Care Co-Ordinator for 16 – 20 hours per week.
The candidate must either have a Bachelors Degree or above or looking for work experience whilst completing your University studies

Essential criteria:
- A full driving license is essential
- Excellent interpersonal skills
- Ability to understand and follow policies and procedures.
- Reliable and honest with good time keeping skills.
- Good English skills both verbally and written.
- Enhanced DBS required
- Either a University Degree or working towards completion of your degree, preferably in Health and Social Care or a medical field
- Experience in Care Planning and Risk Assessments
- Experience in Managing staff
- Experience on using the On Call Phone
About the job and what we would expect from you:
- To provide leadership to care staff within DCK Care.
- To provide and co-ordinate care in accordance with current best practices, according to policy and procedures, agreed standards, legislative requirements, relevant regulations under the direction of the Manager.
- To supervise the care delivery within the service
- To maintain skills at a current level and undertake such training and development as may from time-to-time be required to maintain practices as up to date.
- To complete Rota’s on a monthly basis by communicating effectively with care workers and clients to accommodate.
- To Manage Care workers to ensure working safely, effectively and are happy within their role.
- To work with Clients to ensure they are happy with the care being provided.
- To work complete Client Risk Assessments and Care Plans, updating when needed.
- To work the on-call phone, being a point of contact for emergency situations and cancellations.
- To be able to report and record information effectively onto the system used.
- To complete spot checks, medication reviews and client reviews when needed.
- To be an approachable leader, with open communication with all staff to allow the space for them to develop and work effectively together.
